The SOAP note (an acronym for subjective, objective, assessment and plan) is a method of documentation used by clinicians to write notes in a patient chart. This common format enables healthcare professionals to document in a structured and organized way.
By default, Chartnote displays four text boxes in the Note section.
Subjective or history of present illness.
Objective or physical exam.
Assessment and Plan.
And a miscellaneous area.
You might want to change the number and name of the charting fields on your screen. Depending on your line of work, you might frequently write operative notes or imaging reports, and only need one or two text areas for documentation.
Some clinicians still need multiple sections on their note, but the title of each section might differ from the standard SOAP note.
Whatever your case may be, if you need to edit the charting fields on Chartnote, or just want more real estate in your charting area go to Settings > Preferences > and on Panel Quantity, select the number of text boxes that you want Chartnote to display.
You can then rename the panel titles according to your preferences.
When creating a snippet, you can add placeholders to your template by inserting square brackets. When these placeholders appear in your note they will work as dynamic fields. In other words, the content inside the placeholder can be edited to complete your documentation.
You can enter a description or default value for the field, such as "[date]", leave the field empty, or prompt the content with suggested values.
Navigate the placeholders by saying "next field" and "previous field". To modify the value of the placeholder dictate when the field is selected. To accept the description or default value of the selected placeholder, say "field complete".
If there is no need to change the values in the fields, you can use the voice command “Accept defaults” and all the brackets used as placeholders will be removed from the note.
