This guide explains how the Chartnote + Veracity integration works using Intelligent Mapping and placeholder replacement, and how to set it up step by step.
The Chartnote Veracity integration allows you to map sections of your Chartnote document directly to placeholders inside your Veracity templates using the Chartnote Chrome extension.
Once configured, your notes are transferred instantly into Veracity — structured, clean, and ready to review.

When you open the Chartnote Chrome extension while working inside Veracity:
The extension automatically detects when a Veracity note is open
All your Chartnote notes appear in the extension panel
Your saved mappings are ready to apply instantly
The initial setup is done once per template. After that, your workflow becomes seamless.
Navigate to a patient note or chart inside Veracity.
Once the note is open, launch the Chartnote Chrome extension. The extension will automatically detect the active Veracity note and prepare the integration.
To configure your mapping:
Click Set up mapping in the Chartnote extension
You’ll see:
Chartnote document sections on the left
Veracity patient chart fields on the right
Drag a Chartnote section onto the corresponding Veracity field
You can:
Assign multiple sections to a single field
Remove or change mappings at any time
This flexibility allows you to fully customize how your documentation appears inside Veracity.
Veracity uses placeholders inside templates to insert content dynamically.
To enable this feature:
Click Enable placeholder replacement in the Chartnote extension
Make sure your Veracity template includes placeholders using:
Square brackets: [placeholder]
Or curly braces: {placeholder}
⚠️ Important:
The placeholder names in your Veracity template must exactly match the placeholders used in Chartnote for the replacement to work correctly.
Once your mapping and placeholders are configured:
Click Save and Apply
Your settings are stored for future use
You only need to complete this setup once per Veracity template.
After setup is complete:
Select the Chartnote note you want to transfer
Click Apply Mapping
Your documentation is instantly inserted into Veracity
No extra steps. No manual editing.
Need to make changes later?
At any time, you can:
Open the mapping editor
Update section assignments
Modify placeholder behavior
Remove or add new mappings
