Fixing Inaccurate or Hallucinated Notes

This guide walks you through the quickest fixes when your generated notes look incorrect, nonsensical, or “hallucinated.” As covered in the video, linked at the end of this article, this usually comes down to two root causes:

  1. Wrong voice language is selected

  2. Your microphone isn’t actually capturing audio

We’ll go through them one at a time.

Issue 1 — Wrong Voice Language Selected



If your generated note looks inaccurate or incorrect, you may have selected the wrong voice language when creating it. Here’s how to fix it:

Step‑by‑Step Instructions

1. Open your note

2. Select the Copilot Tools icon

3. Choose Update Language

4. Pick the correct language spoken during the visit


Pro Tips to Avoid Future Issues

  • Create a test note with the correct voice language selected. This choice will stay active for future notes unless changed.

  • If you work in multiple languages:

    • Use Automatic Language Detection if you often switch between visits.

    • Use Multilingual if you speak more than one language during the same visit.


Issue 2 — Microphone Not Capturing Audio


If your mic isn’t being captured, the transcription engine has little or no input to work with. This often results in repeated phrases, very short notes, or empty transcripts.

Step‑by‑Step Instructions

1. Click the View site information icon

This is usually found in the browser’s address bar, next to the URL.

2. Show microphone permission details

Click on the microphone section to view and manage permissions.

3. Open the microphone selection menu

This will let you choose from the list of available input devices.

4. Select the right microphone

Pick the device you want to use (e.g., headset mic, external USB mic, or built‑in laptop mic).
