How to Fix Empty Notes or Hallucinations

If your Chartnote note comes out empty or includes details that weren't part of your visit (often called a hallucination), the cause is almost always one of three things: a microphone or audio issue, the wrong Voice Language selected, or a telehealth setup where Chartnote isn't capturing your patient's audio. The good news is that each of these is quick to identify and fix. Work through the three checks below to get your AI Scribe capturing accurate, complete notes again.



When a note is empty or contains hallucinations, it means Chartnote either didn't receive clear audio or misinterpreted the audio it received. Work through the three checks below in order until your notes are recording correctly.

1. Check your microphone and audio

Empty notes are most often caused by Chartnote not receiving any audio at all, while poor audio quality can lead to hallucinations.

  1. Confirm that your device's microphone is connected and turned on.
  2. Make sure your browser or the Chartnote app has permission to access your microphone.
  3. Verify that the correct microphone is selected if you have more than one audio input device.
  4. Speak at a normal volume and reduce background noise where possible.

2. Confirm the correct Voice Language is selected

If the wrong Voice Language is selected, Chartnote may transcribe your audio inaccurately or produce content that doesn't match what was said, which can appear as hallucinations.

  1. Open your Voice Language settings.
  2. Confirm the selected language matches the language you and your patient are speaking.
  3. Update the language if it doesn't match, then start a new recording.

3. Adjust your settings for telehealth visits

During a telehealth visit, Chartnote may capture your audio but miss your patient's audio if the recording mode isn't set up correctly. This can result in incomplete notes or hallucinations.

  1. Before starting your telehealth visit, select the correct recording mode for telehealth rather than in-person.
  2. Make sure Chartnote is set to capture both your audio and the audio from your patient's side of the call.
  3. Confirm your settings match whether you're using a web-based app or a desktop app, as the setup differs between them.
