If your Chartnote note comes out empty or includes details that weren't part of your visit (often called a hallucination), the cause is almost always one of three things: a microphone or audio issue, the wrong Voice Language selected, or a telehealth setup where Chartnote isn't capturing your patient's audio. The good news is that each of these is quick to identify and fix. Work through the three checks below to get your AI Scribe capturing accurate, complete notes again.
Empty notes are most often caused by Chartnote not receiving any audio at all, while poor audio quality can lead to hallucinations.
For detailed steps, see Troubleshooting Audio and Microphone Issues.
If the wrong Voice Language is selected, Chartnote may transcribe your audio inaccurately or produce content that doesn't match what was said, which can appear as hallucinations.
For full instructions, see How to Change Your Voice Language.
During a telehealth visit, Chartnote may capture your audio but miss your patient's audio if the recording mode isn't set up correctly. This can result in incomplete notes or hallucinations.
For complete guidance, see Using Recording Mode: In-Person vs. Telehealth.
If you've worked through all three checks and your notes are still empty or contain hallucinations, reach out to the Chartnote team for further help.
